This MUST be filled out ONLINE and purchased BEFORE you can enroll your dancer in any classes! If you fail to fill this form out prior to enrolling, any classes you sign up for WILL NOT be held until we receive this registration. Your unique password to access the class enrollment page will be emailed to you AFTER you have completed the registration and paid the enrollment fee. You will receive your code within 24hrs after completion. The registration must be completed for EACH dancer that will be attending KDA. This is Non-Refundable.
