This MUST be filled out ONLINE and purchased BEFORE you can enroll your dancer in any classes! If you fail to fill this form out prior to enrolling, any classes you sign up for WILL NOT be held until we receive this registration. Your unique password to access the class enrollment page will be emailed to you AFTER you have completed the registration and paid the enrollment fee. You will receive your code on August 26th, 2025. The registration must be completed for EACH dancer that will be attending KDA. The registration fee is refundable up to September 1st and must be requested by email.
